Cystic Fibrosis (CF) and Fertility

Cystic fibrosis (CF) is a genetic condition that primarily affects the lungs and digestive system, causing thick mucus buildup, infections, and blockages. It is a recessive disorder, meaning a child will only be affected if both parents carry and pass on the mutated CFTR gene. Around 1 in 20–25 people is a carrier, usually without showing symptoms.

How Cystic Fibrosis Is Diagnosed

  • Sweat Test: Measures chloride levels in sweat, which are abnormally high in CF.
  • Genetic Testing: Identifies CFTR gene mutations. This is highly recommended for couples planning a family.
  • Prenatal Testing: Amniocentesis or chorionic villus sampling (CVS) during pregnancy can detect CF in the baby.

Fertility and Cystic Fibrosis

In Men

  • Most men with CF have azoospermia (no sperm in semen) due to a missing vas deferens.
  • Despite this, sperm can be retrieved directly (e.g., through TESA) and used in IVF with ICSI, making fatherhood possible.

In Women

  • Women may face fertility challenges from thicker cervical mucus or health complications.
  • Many can still conceive naturally with proper medical support.
  • During IVF, genetic testing and Preimplantation Genetic Diagnosis (PGD) help ensure only embryos free of CF are selected.

Ensuring Healthy Offspring

For couples where one or both partners carry the CFTR mutation, PGD offers the chance to select healthy embryos, reducing the risk of passing CF to the next generation and providing peace of mind for parents.
